The causes of varicocele.

Varicose veins of the spermatic cord are the result of insufficient or congenital valve regurgitation of the testicular veins. This condition causes degenerative changes in the testicles. Blood returns from the renal vein through the nuclear vein to the testes, carrying a large number of testicular hormones, such as catecholamines, cortisol, and renin. 

The abnormal structure of the venous vessels in the testicles impairs the blood flow that remains in the veins, putting excessive pressure on the flagellar plexus and causing varicose dilation.

Three factors can influence the development of varicocele.

  1. An increase in hydrostatic pressure in the veins that drain blood from the testicles, which is associated with anatomical differences in their course on the right and left side of the body.
  2. The anterior structure of the vessels is common, so most likely an additional element is responsible for varicose veins of the testicles. It may be a failure of the valves in the nuclear vessels or the presence of additional collateral circulation, which manifests as retrograde venous flow.
  3. Occurrence of the "nutcracker" phenomenon, which consists of the blockage of the left renal vein between the aorta in the back and the superior mesenteric artery. This transfers the increased pressure to the flagellar plexus. There may also be increased pressure in the seed veins due to compression of the common iliac vein between the iliac artery and the edge of the pelvis.

Diagnosis of varicocele
